Don’t forget your insurance card and a photo ID

If you have any questions, please feel free to contact the office during our regular business hours -  Monday to Friday from 8:30am to 4:30pm and the first Saturday of each month from 9am to 12pm.

ALL PATIENTS HAVING AN INJECTION PROCEDURE MUST BE ACCOMPANIED
BY A DRIVER.
If unable to be accompanied by a driver, please contact the office.
